INSTRUCTIONS FOR PROFESSIONAL DEVELOPMENT DOCUMENTATION

Attached you will find the updated Professional Development Activity/ Confirmation Form. Please use this form for any workshop/conference/meeting you want to attend.

Fill out the top of the form as you have
in the past.

Fill in the amount the activity
costs.
IF IT'S FREE, PLEASE ENTER $0.
· Please get your building principal’s signature of approval. Make a copy for your records.
· If a substitute is needed, you are responsible for verifying with Nan at the Switchboard that a substitute has been found for you.
· Please send the completed PD form to the Personnel Office for approval at the Central Office level.
· Once it has been approved you will receive an email confirmation that it was approved. You will no longer receive a copy of the form back, unless the hours are for Section 1526 (see below).
IF YOUR PD ACTIVITY IS CANCELLED OR YOU CAN NOT ATTEND,
IT IS YOUR RESPONSIBILITY TO
NOTIFY NAN OR CAROL AT THE SWITCHBOARD
SO THEY CAN CANCEL YOUR SUBSTITUTE.
For Section 1526 Only (if it is marked “Other” this does not apply):
· Once you have attended the workshop/conference, please either have the presenter or yourself sign the bottom portion of the form (Confirmation of Workshop Attendance) and send the entire form to Keri Goupil in the Personnel Office within one week of the attended workshop/conference. This is important because all hours of professional development will only be credited if this form is received in the Personnel Office.
